Healthcare Travel Costs Scheme (HTCS) and other entitlements

The Healthcare Travel Costs Scheme is for patients on low income, in receipt of benefits or on special allowances to help with travel costs to providers of NHS treatment or tests where patients are referred by an NHS doctor, dentist or specialist.

Each provider of health services referred to by a NHS doctor or dentist determines its own rate of compensation and system.  Enquire at NHS provider or find out more about HTCS here

Summary of procedure:

If you are on benefits: use HC5 claim form from the hospital cashier on the day of treatment.  It is also possible to claim retrospectively for 3 months' previous travel and mailing the HC5 form.

If you are not on benefits but on low income: use Form HCI for all or some help towards travel costs.  HC1 forms are available from NHS outlets such as opticians, hospitals, dentists, as well as Jobcentre Plus.

Please check with local booking clerk or cashier if in doubt:
